Dr. Mekawi Letter of Recommendation Form

I look forward to writing you a letter of recommendation! To make sure that the letter I write is as meaningful, accurate, and helpful to you as possible, I ask that you please complete each item on this form carefully. Please let me know via e-mail (or simply forward the completion email) when the form is completed. I will use the Google sheet you created to update the status of my letters, meaning that when I submit each letter, I will update the spreadsheet ASAP so that we're on the same page.

Please note that this form must be completed at least 30 days before the first LOR is due!
